Step 2 – COMPLETE FORMS

All members, staff, and volunteers must complete forms.

• Complete Acknowledgement of Policies
• Complete Waiver (for all members, staff, volunteers)
• Complete Waiver (for parents with minors in the corps)

Step 3 – MEDICAL INFORMATION

All members, staff, and volunteers must submit medical information
